Bob and Melinda Blanchard's D.R.E.A.M system makes it easy to set a goal, create an action plan and follow through. I have found that making an actual list (not a mental script that you replay daily) of items you want to accomplish will allow you to prioritize them and, more importantly, mark them off when they are complete.
